Qualifications For License

Be over the age of 21 years.

Be a US citizen or legally entitled to remain and work in the USA.

Have graduated from an ADA accredited dental school.

Be of a good moral character:
- Informing whether he or
she has applied for a license in another state giving the date and the results.
- Informing whether he or she has terminated or attempted to terminate such license.
- Providing certification by the licensing authority where
he or she has or is practicing regarding whether his or her license to practice has been suspended or revoked or if there is any pending disciplinary action or is in good standing.
- Submit fingerprints and written authorization to request a FBI report regarding
he or she has ever been convicted of a crime involving moral turpitude or has entered a plea of nolo contender to a charge of such
crime and the final sentence.
- Providing a letter from the dental association of the city or county where
he or she last practiced certifying to his or her moral character.
- Providing the date and address of the last physical exam and the name of the physician who performed that examinations.
- The application will be rejected if the applicant has: a felony or one or more misdemeanor convictions, 2 or more malpractice judgments against, 2 or more peer review actions with adverse recommendations, history of substance abuse or infectious disease, been refused to take a licensing examination or denied a license in another state.

Have passed the NDBI
and NDBII
with a score at least 75.

Be examined on his practical knowledge of dentistry including a clinical demonstration of the
applicant's skills.
The application must be completed at least 45 days before the first day of
examination.

Proof of current malpractice insurance and CPR (Cardio Pulmonary Resuscitation) certification
are required.
Restricted License
The Board shall issue one , without examination to a person who:

Has a valid license from another State.

Has received a degree form an ADA accredited Dental School.

Has a contract with a facility that provides services exclusively to low income persons, under the supervision of a licensed dentist.

The person must pass the Board examination within 3 years after receiving the restricted license.
-
The Board may revoke the license at any time.
Specialty Certificate
A qualified specialist wishing to limit his or her practice to a recognized specialty may apply for a specialty certificate upon passing the general dentistry exam.
Examination
Is administered at least twice annually utilizing dental school facilities in California and is completed in a
four-day period.
The exam is performed in the presence of a quorum of the examining members of the
Board, which provides a dental chair, delivery unit, dental light, operator and assistant stools. All other
equipment and instruments are responsibility of the candidate and
he or she can bring one dental assistant per procedure. The clinical exam includes:

Fabricating a set of upper and lower complete dentures from final impressions to final insertion.

Restoring a tooth using a partial veneer restoration covering the entire occlusal surface while maintaining sound enamel on a buccal or lingual or both of these surfaces of the tooth.

Restoring a posterior tooth using a Class II amalgam restoration.

A closed book jurisprudence examination will be also administered.

Failure Of Examination
Any applicant who fails twice to pass the examination is not eligible for reexamination within 12 months after the second exam was taken.
He or she must complete the additional training required by the Board and update the information of the original application.
DSI Nevada Board courses qualify for this remedial training.
Anesthesia Permit
A permit is required to administer conscious sedation, deep sedation or general anesthesia.
License Renewal And Continuing Education
The license must be renewed every year. 12
hours of continuing education per year are required to practice in Nevada.
A maximum of:

6 from correspondence or videotaped
courses. DSI offers you videotaped courses.
-
3 hours will be accepted for attendance to meetings or conventions (1 hr. for each meeting).

3 hours will be accepted for approved study by a group.
-
4 hours in cardiopulmonary resuscitation or other medically acceptable means of maintaining basic life
support. (The dentist shall attend annually a course of this type).
The course providers must be:

Approved by the Board, ADA or the Academy of General Dentist or any nationally recognized association of specialist.
-
Given by any university, college or community college or ADA accredited hospital.
